At PURE Movement Ltd, the Movement Coach (the Service Provider), will endeavour to meet the Client’s specific wants and goals but will always initially prioritise the Client’s health and physical needs first, E.g. mobility or pain issues. This may include but is not limited to an initial functionality assessment, exclusion from specific exercises, and correction of certain movements, areas, and exercises, prior to starting a specific ongoing exercise programme. Please note, that this may affect the date that independent use of gym equipment for ‘Gym membership access’ is granted.

 

CONSENT TO ASSESSMENT AND TREATMENT

Assessment and sessions at PURE Movement may include but are not limited to physical training (including but not limited to resistance training, corrective exercise), massage therapy techniques, and electrical muscle stimulation modalities. I (the Client) understand it is the policy of PURE Movement to ensure that I am educated about the benefits, side effects, and potential complications of each of the treatment methods used by the Movement Coach. I understand that, during my exercise programme, physical touching and positioning of my body by the Movement Specialist may be necessary to assess my muscular and bodily reactions to specific exercises, as well as to ensure that I am using proper technique and body alignment. Permission will be requested in each instance.

SOCIAL MEDIA

I understand that PURE Movement uses social media for marketing and as an educational tool. Movement Coaches may take group photos or videos of the Client/s for use in promotional material or online (including but not limited to website, educational case studies, and social media outlets such as, but not limited to, Instagram, YouTube, and/or Facebook). Any close-up photos of individual members will require permission from the customer to be published publicly. I understand that when posted online, this information is in the public domain. Should I not feel comfortable having personal information released to the public, I have the right to decline any request of this nature by my Movement Coach. I understand I can terminate any past, present, and/or future social media content regarding me and any affiliates upon request at any time, acknowledging the potential difficulty of completely erasing distributed content once it's been released. Unless specifically requested not to by the client for marketing reasons, all client identifying information is removed as much as possible.

PRIVACY POLICY

Your privacy is of critical importance to us. We collect information about your health history, your physical condition and function, and other related details to help us assess what your needs are, advise you of your options, and provide safe and effective service. All information provided will be kept confidential (with an exception to payment information in ‘Section 4: Direct Debit Authority’, which is provided to a trusted third-party company), unless by the request of the client to distribute, or as required by law. All information is kept in secured storage only accessible by PURE Movement Ltd. Your permission is required to release any of your treatment details, and for us to obtain information from your previous/current health care providers.

RELEASE OF LIABILITY

I am fully aware that I am participating in these sessions at my own risk and will not hold the Movement Coach referenced above, or PURE Movement Ltd responsible in the event of my incurring an injury or exacerbating any previously existing conditions. If I have any medical conditions, I have consulted with my physician to make sure that physical training and other services mentioned above is appropriate to participate in. I understand that it is my responsibility to fully disclose to my Movement Coach any health issues or medications that are relevant to participation in an exercise programme, inform the Movement Coach if there are any activities with which I do not feel comfortable, to cease exercise and report promptly to my trainer any unusual feelings (e.g. pain, chest pain/discomfort, nausea, difficulty breathing, apparent injury, etc.) and to clear my participation in an exercise programme with my physician.

ATTENDANCE AND RESCHEDULING

Clients are expected to attend every scheduled session. A minimum of 24 hours’ notice must be provided to reschedule a session (with the exception of falling sick) and will be rescheduled at a time mutually suited to the Client and Movement Coach. The weekly Direct Debit will proceed, even if a rescheduled session no longer falls within that week. Any rescheduled training sessions are to be used as soon as possible and will expire four weeks after the originally scheduled date unless under special circumstances, at the discretion of PURE Movement Ltd and arranged in writing with PURE Movement.
The subscription and payments can be placed on a holiday hold for up to three weeks per year, with at least two weeks’ notice provided. This is designed to encourage lifestyle balance flexibility where required, accountability, and results, as well as offering a subscription rate for the dependability of ongoing contracted subscription payments. 
A session may be lost if canceled within 24 hours of the scheduled appointment time; it will be at PURE Movement Ltd’s discretion to reschedule. Late arrivals will be seen for the remainder of the scheduled appointment time only. 
If the Movement Coach fails to attend a session at the agreed time, the Client is entitled to request a complimentary training session at an agreeable time. If the Movement Coach cancels a Session with at least 24 hours’ notice, the Session is void and shall be rescheduled on a different date.

PAYMENTS

Payments are required via Ezypay.com (www.ezypay.com). You agree to ensure sufficient funds are available on your chosen withdrawal date. At any time, you can request more information from PURE Movement on Stripe.

TERMINATION

At least 14 days' notice must be given to cancel or terminate this agreement and cancellations must be given in writing. 
50% of the remainder of the agreement must be paid within 30 days of termination notice given, not surpassing the final termination date of this agreement. There will be no refund of any monies paid.

MEMBERSHIP STUDIO ACCESS

Membership studio access will be available once the client has been cleared by the Movement Coach, by means of a Functional Movement Screen, to use specific equipment unsupervised and follow a set exercise plan only. The client may at any time decrease weight/reps or have a break. You may contact a Movement Coach at any time if you have any questions about your exercises or wish to increase your weights or reps. PURE Movement Ltd will not hold any responsibility or liability of injury or otherwise if you deviate from your set exercise plan whilst unsupervised. Premises accessibility hours can be granted with prior arrangement. The studio is generally staffed during the day only. By accessing the studio after hours, you agree to respect the space and other members. The culture of PURE Movement is friendly and welcoming to all, with zero tolerance for harassment, intimidation, and discrimination.  There are security cameras in place.

PERSONAL 

Personal effects brought to PURE Movement’s premises are the Client’s responsibility. PURE Movement and the Movement Coaches are not liable for any loss or damage to the Client’s personal effects or property. 
Clients will inform PURE Movement of any changes to contact details as quickly as possible.
Clients are physically and mentally competent to complete an exercise program and have been cleared to do so by a GP or physician. Injury is unlikely; however, PURE Movement and the Movement Coaches are not liable for any injuries obtained whilst in a program.
